We serve Medical Assistance recipients in Minnesota who have
✔ physical or mental disabilities,
✔ a substance use disorder,
✔ or are age 65 and over.
How we help...
We help you find, pay for and move into housing of your own choosing.
We support you in maintaining housing while moving forward with a fulfilling and meaningful life.
We provide early intervention in situations that may jeopardize housing.
To accomplish this...

We use our strengths to support you in building on your strengths.
We nourish our communities by fostering mutually beneficial relationships.
